Program Specific Job Description:

1)Direct Clinical Care & Case Management

Provides professional therapeutic services topersons with mental illness and/or addictive disease who have Medicare coverageor whose disability is intractable or difficult to manage.  Arranges appointment schedule at optimalintervals to maximize use of work time. Conducts face-to-face comprehensiveassessment to identify problems and service needs. Provides individual, familyor group therapeutic counseling and/or educational and skill developmentsessions as indicated in the service plan. Monitors client progress and compliance with the service plan. 

2)  Crisis Management

Provides crisis intervention services toexisting and walk-in clients as needed. May provide on-call, after-hours services on a rotational schedule.Assesses consumer's need for emergency admission certification.  Executes the legal forms required to referthe client to an emergency receiving facility for evaluation of the need forinvoluntary commitment to a hospital or crisis intervention facility.  Arranges transfer of the client to thereceiving facility.

3)  Clinical Documentation

Writes individual service plans that meetMedicaid and other required standards for diagnoses, content, justificationsfor treatment, evidence of client input, and treatment time frames. Writesservice plans that are clear, concise, complete, grammatically correct, timely,and properly coded.  Secures or providesnecessary signatures required to secure authorization for services.  Follows all Medicaid, state, insurer, andother regulations related to the credentials required to provide authorizedservices.  Writes timely progress notesthat relate directly to the treatment outcomes described in the serviceplan.  Assembles, analyzes, andinterprets new/updated information about the client in order to reassesstreatment needs.  Identifies unmet needs,reviews available resources, and modifies the treatment plan as indicated. Requeststreatment authorizations and re-authorizations within required timeframes.  Provides and bills only thoseclient services that are authorized in the service plan.

4)  Case Management, Referrals, Collateral &Treatment Team Contacts

May serve as case manager to facilitate andensure communication, linkage, and continuity of care between clients,physicians, other organizational components, community resources and service providers.Establishes linkages and coordination of services with community agencies.  Develops and maintains cooperative workingrelationships with community service providers. Informs clients and families ofthe availability of community advocacy and support groups.

 
Individual Job Description:
  • Flexible8 hour shifts available between 8am – 7pm Monday through Friday
  • On–call(after hours) rotation
  • Working with SA & MH Populations
 
Qualifications:
  • MinimumTraining & Experience:
    • Master's degree from an accreditedprogram
    • Georgia professional license as eitherLAPC, LMSW or LPC, LCSW
    • ONE year of supervised experience inmental health and addictive diseases
  • Mustpass criminal records fingerprint check.


Master's degree in a related area from an accredited college or university AND eligibility to be licensed AND Two years of supervised experience in the area of assignment.

Qualifications


Preferred Qualifications:
•LCSW or LPC with 3 or more years of experience working in a Community Mental Health outpatient setting.
•Experience providing counseling, crisis intervention and assessments to adults with SPMI.
•Training and experience in evidence-based treatment modalities
•Knowledge/use of Carelogic or other ECR
•Prior clinical experience working with children, adolescents, and adults.
