Using the cognitive skills of application, analysis, synthesis, and evaluation, the Home Health Medical Social Worker Therapist is responsible for using the concepts of case management to provide, coordinate and supervise medical social services of all patients: neonate, infant, child, adolescent, adult, and geriatric. Key functions include providing short-term counseling, patient/family teaching, patient/family support and advocacy. Accountability will be monitored through direct observations; achievement of service goals including satisfaction, quality, and financial outcomes; and performance improvement activities.
Master's degree from a school of social work approved by the Council of Social Work Education.
Current LCSW or LMSW license in the state of Georgia or current LMSW or LISW license in the state of South Carolina depending on the position location.
Valid driver's license and current automobile liability insurance required. Report of Motor Vehicle Record for past 5 years required.
Normal experience gained in MSW program may count as the one year MSW experience requirement. Previous home health care experience is strongly recommended. Additional hospital or social services experience in other settings is also a plus.

Social services knowledge of growth and development relevant to the ages of the patients served.
Social services knowledge of the diseases and health care needs of the patient population served.
Assessment, analytical, and synthesis skills sufficient to interpret and analyze patient's psychosocial data; prioritize needs; and develop an individualized plan of care.
Evaluation skills adequate to evaluate patient response to care and take appropriate action.
High level of interpersonal skills in order to effectively communicate with patients, families, physicians and other home health care personnel with whom daily interactions must take place.
Problem solving skills sufficient to allow effective problem/ conflict resolution.
Ability to function with minimal supervision.
Ability to provide reliable means of transportation.
Demonstrated traits of initiative and dependability required.
Ability to perform efficiently and effectively under stress.

Ability to be flexible.
Standing for long periods of time (6-8 hrs.). Sitting for extended periods of time (at desk telephoning, making schedules, writing reports).
Bending and stooping (as needed to provide services, treatments and preventative procedures as required).
Walking for long periods.
Lifting of supplies (supplies weighing approximately 25 pounds).
Sensory: sight to observe patient symptoms auditory to hear patients tactile to feel patient symptoms (warmth, induration) olfactory to smell disease specific smells
